Serhiy M. Kozachuk

Serhiy M. Kozachuk, 39, of Liberty Twp., passed away in the Emergency Room of St. Elizabeth Medical Center from injuries sustained in an auto accident on Sunday, August 30, 2015.

Serhiy was born on June 22, 1976 in Ternopil, Ukraine, as son to Michael and Faina (Maydokina) Kozachuk.

Following his high school graduation, he went to trade school to study to become an electrician. He also served his country in the Ukrainian National Army.

He fell in love with the former Iryna Perih and on November 25, 1998, they became husband and wife. Together they traveled to the United States and he found employment as a machinist for PSK Steel.

He was a member of the Holy Trinity Ukrainian Orthodox Catholic Church and in his spare time, Serhiy enjoyed American Football, playing soccer, tennis swimming, and exercise at the Jewish Center. He loved life in America.

He is survived by his wife, Iryna Kozachuk; son, Ostap Kozachuk, Ukraine; brother, Alexander Kozachuk, Ukraine; in-laws, Michael & Lubov Perih, Ukraine; brother-in-law, Olah Perih, Girard; and many many close friends and co-workers.

He was preceded in death by his parents.

Calling hours will be Tuesday, September 8, 2015 from 4-7 PM at Blackstone Funeral Home, 672 Churchill Rd., Girard, with a Panachida Service at 6:30 PM. His funeral Mass will be on Wednesday, September 9, 2015 at 1:00 PM at the Holy Trinity Ukrainian Orthodox Catholic Church, 526 W. Rayen Avenue, Youngstown, conducted by Rev. Fr. Lubomyr Zhybak. He will be laid to rest in Belmont Park Cemetery.
